Pan-Canadian Health Data Content Framework (PCHDCF)

2 relevant standards and additional supporting architectural products are part of the Pan-Canadian Health Data Content Framework:

  • The Data Content Standard of the PCHDCF defines essential data elements and value sets necessary for Connected Care. It specifies the types of data to be collected (e.g., diagnoses) and the languages and formats to be used (e.g., SNOMED CT codes). This ensures that data is accurate, compatible, uniform and consistent across different systems. Value sets, which are lists of specific values assigned to data elements, ensure standardized and meaningful data exchange, improving data quality and reusability.
  • The Canadian Core Data for Interoperability (CACDI) is a subset of the PCHDCF, which aims to define a standardized set of essential health data elements and associated value sets in the context of a common architecture to support interoperability and data exchange across the Canadian health care ecosystem. The CACDI works in tandem with Canada Health Infoway’s CA Core+ to facilitate interoperability and data exchange in the Canadian health care landscape.
  • The Common Data Architecture is a collection of products that help to standardize how data is structured, exchanged and understood across different systems. These products are essential for semantic interoperability, which ensures meaning is preserved when data is shared. These products include a common logical data model, business glossary, business context model and use cases.

Pan-Canadian Primary Health Care Electronic Medical Record (EMR) Minimum Data Set An essential component of improving the standardization and comparability of primary health care data. Defines a focused set of primary health care EMR data elements and associated code systems. SNOMED CT CA to classifications mapsCIHI has created a comprehensive map from SNOMED CT CA to ICD-10-CA. These classification maps can be incorporated into system software to provide a semi-automated link from clinical information recorded by the clinician in an electronic health record using SNOMED CT CA to ICD-10-CA codes. In addition, SNOMED CT CA to ICD-9 maps provide a standardized approach for collecting primary care health concern information and support patient-level physician billing.Updated quarterlyYes*
