Ann: Connected Care sees a future in which patients don’t have to repeat their prescriptions or their medical history to every health care provider they see. It is also a future in which patients and their families have a meaningful role to play in their treatment and ongoing care. The path to this is information flow. That is the work that CIHI and Canada Health Infoway are working on together to ensure that health care providers have access to accurate and consistent and comprehensive information about their patients to ensure safe and effective care.
