High Users of Hospital Beds

This indicator provides the risk-adjusted rate of patients who had 3 or more inpatient acute care hospitalizations with a cumulative length of stay (LOS) longer than 30 days (high users).

Hip Fracture Surgery Within 48 Hours

This indicator calculates the risk-adjusted proportion of hip fractures surgically treated within 48 hours of a patient’s initial admission to an acute care hospital, among patients age 18 and older.

Hip Replacement Rate

This indicator measures the hospitalization rate for hip replacement procedures performed in acute care hospitals or same-day surgery facilities per 100,000 population age 18 and older.

Home Care Services Helped the Recipient Stay at Home

Percentage of households where at least one member received home care services in the past 12 months and reported that services were very helpful in allowing the recipient(s) to stay at home.

Hospital Deaths (HSMR)

The ratio of the actual number of in-hospital deaths in a region or hospital to the number that would have been expected, based on the types of patients a region or hospital treats.

Hospital Deaths Following Major Surgery

This indicator measures the rate of in-hospital deaths due to all causes occurring within 30 days of major surgery.

Hospital Harm

This indicator measures the rate of acute care hospitalizations with at least one occurrence of unintended harm during a hospital stay that could potentially have been prevented.

Hospital Stay Extended Until Home Care Services or Supports Ready

This indicator measures the median number of days patients spend in an inpatient acute care hospital bed when they don’t need acute care before they are discharged to home care.

Hospital Stays for Harm Caused by Substance Use

This indicator calculates the age-standardized rate of hospital stays for harm caused by substance use per 100,000 population age 10 and older.

Hospitalizations Entirely Caused by Alcohol

This indicator provides the age-standardized rate of hospitalizations with conditions that are wholly (100%) attributable to alcohol, per 100,000 population age 10 and older.

Hospitalized Heart Attacks

This indicator measures the age-standardized rate of new acute myocardial infarction (AMI) events admitted to an acute care hospital for the population age 18 and older.

Hospitalized Hip Fracture Event

This indicator measures the age-standardized rate of new hip fractures admitted to an acute care hospital, per 100,000 population age 65 and older.

Hospitalized Strokes

This indicator measures the age-standardized rate of new stroke events admitted to an acute care hospital for the Canadian population age 18 and older.

Hysterectomy Rate

This indicator measures the age-standardized rate of hysterectomy provided to patients in acute care hospitals or same-day surgery facilities, per 100,000 women age 18 and older.

Improved Physical Functioning in Long-Term Care

This indicator provides the percentage of long-term care residents who improved or remained completely independent in transferring and locomotion.

In-Hospital Sepsis

This indicator calculates the risk-adjusted rate of in-hospital sepsis per 1,000 discharges.

Incidence of End-Stage Kidney Disease (ESKD), by Primary Diagnosis

This indicator gives the breakdown, by primary diagnosis, of all patients with end-stage kidney disease (ESKD) who began renal replacement therapy (RRT) for the first time in the calendar year.
