ICD-11 change request form

This form guides you through the process of submitting change proposals for ICD-11 to the World Health Organization (WHO) through the WHO-FIC Maintenance Platform.

Providing detailed information about and justification for your proposal will facilitate its passage through the decision-making process. CIHI’s Classifications and Terminologies team can help assess the completeness of your proposal. We can provide additional guidance as you submit your proposal on the WHO’s platform.

There are 5 proposal templates on the WHO’s platform. Below is a short description of each one. To have CIHI’s Classifications and Terminologies team assess your proposal before you submit it to WHO, complete and submit 1 form for each proposal type.

1) Content enhancement

Addition / deletion of a synonym
Addition / deletion of an exclusion
Change of description (definition)
Change of a title (the request cannot change the meaning)
Correction of spelling or grammar
Move an existing index term to a new code in the ICD-11 MMS(Change of Coding Status)

2) Add a new entity

Add an entity (e.g., a new disease) to the foundation only
Add a unique code for the entity in the ICD-11 MMS

3) Delete an existing entity (index term or code)

Remove an existing entity (index term) that isn’t a code from the ICD-11 MMS
Remove a code from the ICD-11 MMS

4) Complex hierarchical change

(changes to the block structure of a chapter or more complex changes than described in the other request types)

5) Postcoordination

(changes to postcoordination rules and/or value sets at a particular ICD-11 MMS code)

Content enhancement

Complete sections a to f, as applicable.

a) Addition/deletion of a synonym of a current entity

Explain which synonym you want to add or delete, and why.

b) Addition/deletion of an exclusion

Explain which exclusion you want to add or delete, and why.

c) Change of description (definition)

Explain the change by using strikethrough for a deletion and underline for an addition. Provide your rationale to support the change.

d) Change of a title

Explain the change you’re requesting, and why. Note: Changes to a code title that affect the meaning of that code are not allowed and will not be accepted. If a concept is outdated and must be updated, or if a new concept is necessary, you must use the forms to add a new entity and/or delete an existing entity.

e) Correction of spelling or grammar

Explain the error.
Propose the correction.

f) Move an existing entity to a new code in the ICD-11 MMS

Indicate which entity you propose to become a code.
Provide rationale and evidence to support a new code in ICD-11 MMS. Include clinical resources/references

Complex hierarchical change entity

Explain the complex hierarchical change.

Provide your rationale and evidence to support why the complex hierarchical change should be considered.

Postcoordination

Explain what changes you request to postcoordination rules and/or value sets at a particular ICD-11 MMS code.
